Please allow for up to 7 working days for completion of your medical forms. Only forms pertinent to medical problems which are actively being treated by Theresa Fynn, MD and Kwame Ntim, MD will be completed. Forms not related to medical problems being treated by this office should be referred to your primary care physician or the medical office which is caring for that medical problem.
We will be happy to fax any medical records to any medical facility that is related to your care or transfer of care, free of charge. There is a charge for medical records given directly to you. Please call our office to find out applicable fees.
Please click here or call our office at 407-593-2910 during office hours to schedule an appointment. If your insurance plan requires a referral, appointments must be made for your office visits. If you are a self pay or your insurance plan does not require a referral, you may come in without an appointment. It is always good to call ahead and find out if the doctor is available. If you need to cancel an appointment, please call at least 24 hours before your appointment or else incur a "no show" fee.

If your Photo ID does not reflect your current address, please bring a utility bill showing your current address.
Your co-pay is required at the time of your visit. We accept cash, check, Visa, Master and Discover Cards. Many major insurance plans are accepted, please call our office to verify your plan's coverage. If your insurance plan requires a referral, appointments must be made for your office visits.
